You will have direct phone access to one specific, highly experienced, Registered Nurse--called a Care Manager-- who will be assigned to you and available to your whole family. Your A1C Care Manager will provide education and resources to support you and your family members with finding the most appropriate and cost-effective care. Over time, your A1C Care Manager will become familiar with you and your health history, and will be your advocate. You’ll have the individualized support you need to make progress in the face of big challenges.
