The A1C program is designed to help members who have more serious health issues, like chronic conditions and acute medical events. For example, if you’re diagnosed with diabetes or high blood pressure or if you’re facing surgery or a hospital stay.
With A1C, you can connect by phone with a registered nurse, called a Care Manager, who can give you one-on-one support for high-risk health situations. You’ll work with the same Care Manager for all of your health issues. Your Care Manager can get to know you and your unique health issues, so you never have to repeat yourself. In fact, you can have the same Care Manager for all of your covered family members, too.
If your claims data indicates you could benefit from an A1C Care Manager, an A1C representative will call to invite you to join the program.
But, you don’t have to wait for a phone call. If you have been diagnosed with a condition, or have an upcoming surgery scheduled, just call Aetna Member Services at 1-877-458-4975, and select Option 5. Member Services can direct you to the appropriate Care Management program, to see if you qualify and/or to get started.
