At an in-network provider, the plan pays 85% of the cost of the discounted network rate, while you pay 15% if you have met your deductible.
At an out-of-network provider, the plan pays 65% of the reasonable and customary cost for a doctor’s visit in your area, while you pay 35% if you have met your deductible.
