If you’re in an active course of treatment with an out-of-network doctor at the time you become a member of the LM HealthWorks Plan, you may be eligible for Transition Coverage.

The LM HealthWorks Plan provides members with access to Condition Management programs. If you have been diagnosed with a condition, or have an upcoming surgery scheduled, just call Aetna Member Services at 1-877-458-4975, and select Option 5. Member Services can direct you to the appropriate Care Management program, to see if you qualify and/or to get started.
