If you’re in an active course of treatment with an out-of-network doctor at the time you become a member of the LM HealthWorks Plan, you may be eligible for Transition Coverage.

Medical Transition of Care

"I've been working with a plan case manager for help with medical treatment decisions. Will I be able to get this same kind of help under the LM HealthWorks Plan?

Yes. The LM HealthWorks Plan Medical Services Team provides members with Case Managers, who are nurses who can help you understand your health plan coverage while working with your doctors and other health care providers. Case Managers can also help you understand your illness and the care your doctor and others have ordered. When you and your doctor have clear information about your health plan coverage, you are in a better position to use your available benefits and resources. The care you receive is still a decision made by you and your doctor. Working with a Case Manager does not change your health care coverage in any way.
